My preference if it comes to frequency is weekly.

Discussions should be documented (I am using One Note for that purpose)

Remember that it's their meeting not yours. Ideally they should do 70% of the talking.

Understand the professional background of the person.

Understand current responsibilities.

Understand how person feels about the work they currently doing.

What are their personal goals? What they want to achieve in the company?

Build personal development plan.

Make sure you know what is causing stress for your team members.
